Overview of Steps & Requirements:

1. Fill-out and submit your application form online.* (A Instructions)
2. Send a print-out of the submitted form with your penned signature. (B Instructions)
3. Submit your photo (picture file). (C Instructions)
4. Complete your application before the Application Due Date.

Important: Your application is NOT COMPLETE without your photo and penned signature!

A. Instructions for filling-out and submitting your application form online:

1. Download the application form at Acrobat.com via this link: http://ucgyouthcorps.org/2009/appfile.htm

2. Fill-up the form as accurately and completely as possible. Fields bordered in red are required. You may include your own digital signature in the signature fields; for help to do this, please click here.

3. Once you have reviewed your form, click the Submit or Submit Form button.

4. In the Send Form box, enter your E-mail address and Full name, then click Send.

DO NOT CLOSE your browser window at this point.

Note: Your application is NOT complete without your penned signature. You will still need to send a printed or scanned copy of the form you just submitted together with your signature. [Please proceed to Part B.]

B. Instructions for sending the submitted form with your penned signature:

5. Click Print (yellow button) at the top of the application form and create a print-out of your submitted form. You may also want to save this form for future reference (green button).
